Writer's Block: A Tip About Tipping

Anyone who gets both me and my daughter-in-law deserves a higher tip, as long as they don't mess it up entirely! We have some very specific requirements.
If the service is spectacular -- or even 1% above average on a very trying (for the servers) night -- I've gone as high as 50% on an $80 ticket.
The only reason I'll NOT tip is if the server is horrid. And I'll be talking with the manager long before it's time to figure a tip. If I have to get a server replaced (and it has happened), I make certain that the one who actually did the work gets the tip. I won't leave it on the table b/c often whoever was first assigned is supposed to get it.
I'm really not that difficult as a customer -- I want polite, wisecracking, attentive service. If I want a refill I expect to be able to flag someone down within two or three minor attempts. I do NOT LIKE standing up and heading for the kitchen or the bar -- it upsets people! -- but I've done it! And I do NOT blame the server for kitchen problems, limited menu selections, whatever.
If the server is hanging out gabbing with friends and griping about the lack of a smoking section while several people are trying to get their attention, however, THAT's a problem and I WILL address it. Even at a McDonald's.
